In the event that a fall does occur, our experienced Registered Nurses (RNs) and therapists are trained in wound management, the healing process, and the selection and use of effective wound products and devices. Our clinicians are up-to-date on the latest standards and best practices for wound care, including our comprehensive clinical training program designed specifically to:

- Decrease time spent on dressing changes

Our customized program helps evaluate risks and prevent devastating falls.
Step 1: Evaluation
We’ll evaluate your risks for falling by assessing a range of factors including:
• Strength, flexibility, coordination & balance, including issues that may affect these factors, such as your vision, hearing and inner ear.
• Medications and their effects
• Potential in-home hazards that could lead to a fall
Step 2: Personal Improvement Plan
Once we know your risk factors and have identified their cause, we’ll work with you, your family and physician to customize a treatment program to your unique needs.
Step 3: In-Home Care & Treatment
A ProActive Steps™ health care professional will work with you in the comfort and privacy of your home to address the risk factors outlined in your Personal Improvement Plan and help you to reduce your risks of falling.
