Medications can be complicated. Management doesn’t have to be.
Having multiple medication prescriptions can be overwhelming. Our staff work with clients to ensure they have the medications they need, are educated to take them as prescribed, and provide teaching so clients and caregivers can become more independent with medication management. Our trained nurses will reconcile medications in the home and will coordinate with the client’s providers to make sure everyone in the care team has an accurate list of active medications, resolving any discrepancies or potential contraindications that may occur.

Home Care Interventions
- Perform medication reconciliation in the home including all prescribed and over the counter medications
- Assess for compliance with prescribed medication regimen
- Assess for signs of adverse drug reaction such as toxicity, allergy and/or significant side effects or drug interactions
- Assess for signs of ineffective drug therapy
- Instruct on actions, dosages, schedules, and potential side effects
- Instruct on medication safety
- Evaluate if the use of medication aids such as pill organizers or medication timers would promote independence.
- Encourage and/or facilitate client follow up with physician to help ensure prescription refill timeliness
- Coordinate medication refills and home delivery if needed and where available
- Evaluate if pharmacy pill packs would assist in achieving self-management, coordinating set up when appropriate
- Evaluate for and establish medication dispensing machine management when indicated
